What Are Burn Scars?

  1. Steroid injections – Particularly effective for large, raised scars (hypertrophic scars and keloids). Steroids reduce inflammation and help flatten the scar tissue
  2. Fat injections – Useful for depressed or sunken scars, adding volume to create a smoother surface
Multiple sessions may be needed for optimal results.
Surgical Treatment
Surgery is considered when improvements in both appearance and function are desired and cannot be achieved through conservative methods alone.

Surgery is typically performed only after the burned skin has healed and softened—a process that can take up to 18 months. This waiting period allows the tissue to mature and scar formation to stabilize. However, in severe or highly disfiguring cases, surgery may be performed earlier.
Types of Surgical Procedures for Burn Scars
Several surgical techniques can be used, depending on the specific characteristics of your scar:
1. Skin Grafting
Skin grafting is commonly used to treat scars that have resulted in skin contraction and deformity. Healthy skin from an unburned area of your body (donor site) is carefully harvested and transferred to cover the scarred area. This improves both appearance and function by replacing damaged skin with healthy skin.
2. Flap Surgery
Flaps are used for severe burn scars, particularly on the face or other cosmetically important areas. Unlike simple skin grafts, flaps include blood vessels, so they maintain their own blood supply. This allows for better healing and more natural appearance, especially for complex reconstructions.
3. Serial Excision
For extensive scars, serial excision involves removing the scar in multiple stages over several months. This allows surrounding skin to stretch gradually and cover the area, often producing better cosmetic results than removing the entire scar at once.
4. Tissue Expansion
This innovative technique involves implanting small expandable devices beneath healthy skin adjacent to the scar. Over several months, these devices are gradually filled with saline, causing the skin to stretch and expand. Once sufficient skin has been generated, the expander is removed and the extra healthy skin is used to cover and replace the scarred area. This approach often provides the best cosmetic match since the replacement skin comes from adjacent areas and maintains natural color and texture.

Combined Approaches
In many cases, a single patient may require a combination of these techniques applied at different stages to achieve the best overall result.

When Should Burn Scar Surgery Be Performed?
Surgery is typically performed after the burned area has healed and the scarred skin has become soft and supple. This maturation process can take up to 18 months. Attempting surgery too early, when scars are still firm and immature, can result in poor outcomes.
Exceptions
In severe, highly disfiguring, or functionally limiting cases, surgery may be performed earlier than the typical 18-month waiting period to restore critical function or prevent serious complications.
Complementary Treatments and Care
Along with surgery or conservative management, several supportive treatments enhance results:
1. Physical Therapy (Physiotherapy)
Specialized exercises help maintain and improve mobility, particularly if contractures have limited movement. Physical therapy is essential for optimal functional recovery.
2. Splinting
Custom splints help maintain proper positioning and prevent contracture recurrence, especially during the healing phase after surgery.
3. Pressure Garments
Specially designed compression garments apply controlled pressure to scars, helping to flatten, soften, and reshape them. These are often worn for extended periods post-surgery or as part of conservative management.
4. Scar Management Products
Specialized creams, ointments, and silicone products can help improve scar appearance when used consistently.

Benefits of Burn Scar Treatment
While surgery cannot completely erase burn scars, it can achieve significant improvements:

While scars may not disappear entirely, they can be substantially improved. Raised, thick scars can be flattened; depressed scars can be filled; and overall appearance can be significantly enhanced.
